Case Management

Seniors are often faced with chronic or catastrophic circumstances that stress their coping skills, personal/financial resources, and learning and functional abilities. This can be a daunting experience for elderly persons and their families. Age Well’s case managers help these at-risk individuals understand the varied community services that are available to them, and then guide them through the steps required to obtain those services.

Case managers are available for consultation in each of the senior centers managed by Age Well Senior Services. In-home visits can also be arranged when necessary. Individuals and their families or caregivers receive a professional assessment of critical needs and overall condition, a recommendation for appropriate services, and referrals and/or authorizations to get them started.

There are many professional, life-enhancing, service resources available to senior citizens in South Orange County. But navigating the system to locate and receive them can be overwhelming. Age Well’s case management professionals provide a link that is absolutely vital to allowing frail seniors to maintain their independence while avoiding premature institutional care.
